North Suffolk is seeking Overnight Counselors for our two Substance Abuse/Addictions Residential Programs – Meridian House, located in East Boston, and our new program River House located in Chelsea.  These programs provide behavior modification treatment for individuals diagnosed with substance use disorder and co-occurring disorders. We provide medication assisted treatment for those individuals who meet the criteria and residents are treated for mental health disorders. This is a 24 hour a day program with 24 hour per day staffing.

What you’ll do:

  • Be awake and available to consumers during overnight shift.
  • Perform hourly rounds and monitoring checks.
  • Write log notes at end of shift, noting any incidents, and maintain regular communication with other team members.
  • Observe medication administration.
  • Complete basic administrative tasks such as answering phone for program, filing, data entry, and word processing.
  • Monitor and ensure health and safety of residents.
  • Respond appropriately to residents and act as a role model.
  • Contribute to the efforts of the program in maintaining a high quality living environment, and to the overall quality of life for consumers.

North Suffolk is looking for a Case Manager for our residential recovery program!  The Case Manager provides case management services in a structured residential Substance Use Disorder setting, through education and life skills to help people transition in to sustainable recovery.

What you’ll do:

  • Supervise and/or participate with residents, based on their needs in self-help skills, activities of daily living, socialization, leisure time and community integration.
  • Assist the individual’s team to develop an IAP (Individualized Action Plan).
  • Implement, teach, and assist individuals with IAP goals and objectives on a consistent basis.

North Suffolk is looking for Recovery Specialists for our residential substance use program!  The Recovery Specialist is responsible for the health, safety and quality of life of individuals in a residential setting through advocacy, teaching and service planning, individual and group counseling. This is a 40 hour per week position with no weekend hours and no on-call requirement.

  • Guide and assist residents on meeting treatment plan goals
  • Oversee the milieu to ensure residents are safe and supported

NSMHA is looking for a Clinical Coordinator to join our Hanton House team!  The Clinical Coordinator leads the COE in the assessment, treatment planning and service delivery processes and is responsible for determining persons’ need for rehabilitative services. They oversee the development of the persons’ treatment plan and subsequent reviews and revisions to ensure the goals, objectives and interventions reflect the Clinical Formulation-Interpretive Summary and the prioritized assessed needs. Services are provided based on Recovery tenets and are aimed to promote overall health and well-being of persons served. The Residential Clinical Coordinator provides leadership and assists clinical supervision and guidance to direct care staff.

What you’ll do:

  • At the direction of the Residential Program Manager:
    • Engage people and partner with them to improve their health and wellness, achieve symptom stabilization and self-management, develop and/or restore skills impacted by mental illness/substance use and live a self-directed life.
    • Assist people with psychiatric and substance use disorders to identify personal priorities, preferences, strengths, and interests in order to help them establish goals that support a life in recovery.
    • Provide clinical services in timely and flexible ways that are consistent with the principles and practices of trauma informed care and person centered approaches.
    • Collaborate with persons to help them to write goals with specific action steps in order to develop effective treatment/rehabilitation recovery plans.
    • Participate in the development and implementation of Safety Plans and Crisis Prevention and Response Plans in order to help prevent and mitigate risk factors.
    • Be knowledgeable of and use evidence based practices including motivational interviewing, SBIRT, Harm Reduction, Stages of Change and Housing First and other relevant best practices.
    • Provide supervision, mentoring and modeling of evidence based practices with direct care staff.
    • Assist in provision of skill building interventions that help move a person on to the most independent setting or level of care
  • With clinical guidance, and utilizing the principles of psychiatric rehabilitation and recovery, provide relationship building, life transition, addiction, and family engagement services.
  • Document each person’s progress in the confidential record according to established methods and procedures.
  • Provide and carry out assigned interventions and action steps of the treatment plans.
  • Document the need for rehabilitative services as required by Medicaid Rehabilitation Option.
  • Complete, sign and date the Clinical Formulation-Interpretive Summary and Prioritized Assessed Needs sections of the Assessment to authorize rehab option billing.
  • Organize and lead individual and group social and recreational activities to structure each person’s time, increase social experiences, and provide opportunities to practice social skills and receive feedback and support.
  • Prepare information and testimony related to guardianship hearings, including Rogers type and case summaries in preparation for clinical and risk reviews.
  • Provide active efforts to engage people who have traditionally been difficult to engage in services including outreach to shelters and transitional residential programs and more.
  • Serve as a liaison to courts, including filing for Section 35s.

What you’ll need:

  • Education: Masters level clinician (MSW or Mental Health Counseling required).
  • Experience: Previous experience in the provision of clinical or rehabilitative services to adults with chronic and persistent mental health issues and/or substance use disorders preferred.
  • Skills: Must possess a valid Mass driver’s license and reliable transportation.  Must be MAP certified and CPR/1st aide certified within the first three months of hire.

As a Program Manager you will oversee the administrative, financial,  and operational operations of a group home for adults with developmental disabilities. As the Program Manager you will be responsible for the clinical and program planning, medication administration, laundry, shopping, menu/meal planning and the maintenance and safety of the group home. Liaison with families and funding sources.  This position interfaces with staff, peers, senior management, families, neighbors, medical practitioners, and DDS service coordinators.

Schedule: Monday-Friday 10a-6p

Responsibilities:

  • Recruit, hire, train, assign, direct, counsel, discipline and evaluate performance of the Assistant Program Director.
  • Liaison between clinical community and staff, program, clinical and agency level problem solving.
  • Manage a daily residential schedule, ensuring the program meets Individuals’ needs, and allows for their maximum involvement.
  • Make sure the program is operating per required ratios and are within budget.
  • Meets with direct support team each month to support and motivate them.
  • Oversees staff assigned to Individuals to ensure client’s successful participation in appropriate recreational and community activities.
  • Fosters and maintains positive relationships with funding sources, day rehabilitation programs, families, and neighbors through an appropriate level of communication.
  • Oversees the development of behavior plans when needed and secures required approvals for behavior plan before implementing plan.
  • Trains staff in the implementation of newly approved behavior plans.
  • Position requires on-call coverage and some evening and weekend on-call rotation. Also requires some local travel to accompany Individuals to doctor’s appointments or for medical treatment.
  • Other duties may be assigned.

Requirements

Education: High School Diploma/GED equivalent required. Bachelor’s Degree in a related filed highly preferred.

Experience: Two years of supervisory experience. Two year’s experience in Human Services, specifically experience with population that has developmental disorders and/or mental health, and supervisory experience required.

Skills: Must obtain Massachusetts MAP & CPR/First Aide certifications within five (5) continuous months of hire (training provided). Valid Mass driver’s license required.

Direct Support Professionals will work with individuals who need complete assistance with ADL’s, and/or feeding, behavioral interventions in the home and in the community.  Most individuals served have level 2 behavior plans, and/or aggression and staff will need to complete restraints using techniques learned through NAPPI training. This role is also an ISP specialist and will take the lead writing ISPs and training other staff how to implement ISP goals and summaries.  This staff will be able to train other staff with the assistance of the Behavior Specialist and Program Manager on behavior plans and reward systems.
