North Suffolk is looking for an Assistant Program Manager to join our Community Service Agency!  This position assists the CSA Director by carrying out supervisory responsibilities in accordance with North Suffolk’s policies and applicable laws.  The Assistant Program Manager provides weekly supervision and evaluation of Care Coordinators and recommendations on hiring and discipline via corrective action to Care Coordinators and/or Family Partner staff.

What you’ll do:

  • Interview, select and recommend hiring of employees and make recommendations on the retention, compensation, promotion, layoff and/or recall of employees.
  • Train and support program staff to meet requirements identified in the ICC/FP Performance Specifications and agency policies
  • Complete annual staff evaluations, utilizing independent judgment, for all staff assigned.
  • Discipline supervisees for just cause by utilizing corrective action procedure, with guidance from CSA Director.
  • Independently review and sign off on Initial Clinical Assessments, SCND, Discharge Plans, and all other client documentation
  • Monitor weekly billing tickets of supervisees to ensure that standards are met for ICC/FP services.
  • Receive and triage program referrals as directed. Assign cases, in collaboration with Sr. Family Partner and independently, to staff based on evaluation of case needs and staff capacity.
  • Carry and respond to referral pager on a rotating schedule with CSA Director and Sr. Family Partner.
  • Serve as On-call Supervisor for ICC staff; as dictated in the CSA check out procedure.
  • Participate in weekly Utilization Review of program files.
  • Provide care coordination services to caseload of 2 families including completion of SCND, Initial Clinical Assessment and all other documentation.
  • Attend and assist facilitate monthly SOCC with CSA Director.

What you’ll need:

  • Education:  Master’s degree in the mental health field including, but not restricted to, counseling, family therapy, social work, psychology, etc., from an accredited college or university required.
  • Licensure:  LICSW or LMHC required
  • Experience:  Minimum three (3) years of experience in providing outpatient behavioral health services to youth and families.  Experience with home-based or wraparound model is preferred.  Familiarity with child & youth serving programs with at least (1) year supervisory experience in child and family systems.  Must have experience working collaboratively with state agencies, consumer advocacy group, and/or behavioral health outpatient facilities.

NSMHA is looking for a Family Partner to join our Community Service Agency!  This paraprofessional position provides a structured one-to-one, strength-based relationship to a parent/caregiver of a Seriously Emotionally Disturbed youth for the purpose of addressing the youth’s emotional and behavioral needs.  This is accomplished by enhancing parenting capacity and increasing access to natural community resources to enable the parent/caregiver to parent the youth successfully.

This position is designed for someone with lived experience as a caregiver of a youth with special needs (preferably mental health needs).

What you’ll do:

  • Engage the parent/caregiver in activities in the home and community.
  • Assist the parent/caregiver in addressing one or more goals on the youth’s treatment plan for Outpatient or In-Home Therapy or Individual Care Plan.
  • Coordinate Intensive Care Coordination Services with the Care Coordinator within the agency and Hub providers outside of the agency.
  • Assist the parent/caregiver in meeting the youth’s needs by educating, supporting, coaching, modeling or guiding and fosters empowerment.

What you’ll do:

  • Provide early intervention screening/intake, evaluation and assessment of service needs and provision of appropriate direct services.
  • Provide direct child developmental and family support and home teaching services and case management to clients in home, program, and community settings.

  • Supervise children during program activities.
  • Implement behavior management/modification in the milieu.
