North Suffolk is looking for a Clinician to join the After School Program! CASP is a voluntary, community based, therapeutic program for children ages 6 to 13 with emotional, behavioral, and mental health needs. The program is structured and utilizes a positive reinforcement reward system. At CASP, children participate in therapeutic groups which focus on developing social skills and adaptive coping skills. A primary goal of the program is to support and maintain children with their families in their home communities. To do this, each child is assisted with case management services and coordination of care between the program, family, and other providers. While at CASP, each child works directly with a Clinical Therapist, a Youth Care Specialist and a Youth Care Mentor. Additionally, transportation is provided year round.

What you’ll do:

  • Perform biopsychosocial, multiaxial assessment and diagnosis
  • Formulate and implement individualized consumer-centered treatment plans as part of the treatment team; monitor progress, reevaluate and revise on a monthly basis.
  • Plan and facilitate therapeutic groups (e.g. social skills, coping skills, etc.).
  • Provide behavioral crisis intervention &/or triage with program consumers during a crisis using appropriate techniques.
  • Perform face-to-face outreach visits (in coordination with a teammate) &/or telephonic outreach.
  • Participate in activities of service coordination, consultation, education and evaluation to successfully manage assigned consumer caseload within the team context.
  • Provide liaison between team and collateral providers.
  • Facilitate family consultations &/or multi-family group(s) as per program need.
  • Complete all clinical documentation and prior approval forms required.
  • Oversee administration and documentation of clients’ prescription medications, assuring adherence to medical policies and procedures.
  • Drive pupil van and accompany other staff during ride.

NSMHA is looking for a Family Partner to join our Community Service Agency!  This paraprofessional position provides a structured one-to-one, strength-based relationship to a parent/caregiver of a Seriously Emotionally Disturbed youth for the purpose of addressing the youth’s emotional and behavioral needs.  This is accomplished by enhancing parenting capacity and increasing access to natural community resources to enable the parent/caregiver to parent the youth successfully.

This position is designed for someone with lived experience as a parent or direct caregiver of a youth in their family with special needs (preferably mental health needs).

What you’ll do:

  • Engage the parent/caregiver in activities in the home and community.
  • Assist the parent/caregiver in addressing one or more goals on the youth’s treatment plan for Outpatient or In-Home Therapy or Individual Care Plan.
  • Coordinate Intensive Care Coordination Services with the Care Coordinator within the agency and Hub providers outside of the agency.
  • Assist the parent/caregiver in meeting the youth’s needs by educating, supporting, coaching, modeling or guiding and fosters empowerment.

NSMHA is looking for an Assistant Program Manager to join our In-Home Therapy Team!  This position supervises IHT staff as well as provides direct clinical services for clients.  The Assistant Program Manager is responsible for ensuring the overall coordination of care for clients and ensures that all services are provided in accordance with the IHT/TM Performance Specifications and Systems of Care Philosophy.   The Assistant Program Manager works closely with NSMHA staff, parents/guardians, community providers and referral sources as appropriate and/or directed.

What you’ll do:

  • Provide in the moment and weekly clinical/administrative supervision to clinicians; Training and Support staff; Therapeutic Mentors; and interns, as assigned, to meet IHT/TM Performance Specifications and medical necessity criteria.
  • Independently review and sign off on Staff Safety Assessments, Comprehensive Assessments, Treatment Plans, Discharge Plans, and all other client documentation requiring the signature of an IHT/TM supervisor.
  • Monitor weekly billing tickets of supervisees to ensure that standards are met for IHT/TM services.
  • Receive and triage program referrals as directed.  Assign cases, in collaboration with Program Manager and independently, to staff based on evaluation of case needs and staff capacity.
  • Carry and respond to referral pager and On-Call Leadership on a rotating schedule.
  • Collaborate with providers and referrers to determine, utilizing independent judgment, whether a case might be appropriate for IHT/TM services.
  • Attend and participate in all scheduled meetings with MCEs.
  • Participate in weekly Utilization Review of program files.
  • Provide 1-1 IHT services as an IHT clinician to a caseload determined by Program Manager.
    • Provide crisis intervention and guidance to clients and clients’ families.
    • Formulate independent psychosocial diagnosis and treatment plans.
    • Complete Risk Management Safety Plans.
    • Provide ongoing assessment and treatment to clients using individual, group and family modalities.
    • Accompany families to meetings pertaining to the youth’s behavioral health treatment in a variety of settings. (e.g. school, foster home, community based locations, etc.)
