NSCS collaborates with state and local partners to provide services to individuals who may need more intensive or specialized services, many of which are not covered by insurance. These services include residential, intensive community outreach and case management services for people living with varying abilities and skill levels.
Our community-based services have supported individuals’ independence and dignity for more than 60 years and have allowed us to be pioneers in recovery and community inclusion. We foster independence through skill and resource building to support safer, healthy, meaningful and productive living and ensure people feel connected to their families and neighborhoods.
Our Services
Behavioral Health Community Partners (BHCP) provides person-centered care coordination for MassHealth members with behavioral health, medical and social needs to improve health outcomes and quality of life. BHCP, in partnership with Riverside Community Partners, works with eligible adults across North Suffolk departments and throughout Greater Boston and the North Shore. Our goal is to foster healthcare integration and promote preventative care while decreasing visits to the Emergency Room or hospitals.
For information about BHCP, contact: Jennie Reis, Program Director at jreis@northsuffolk.org.
