Program Coordinator – Conexions

North Suffolk is looking for a Program Coordinator to join our Conexions team! The Program Coordinator position is responsible for the overall clinical administration of the Intensive Outpatient Model of Treatment for Adolescents and Adult in a community setting; also to provide direct behavioral health services to consumers, and to supervise the delivery of the same by other clinicians as an integral part of the program’s multidisciplinary team. They work directly with the Access and Engagement team promoting SUD expertise to the rapid access process.

What you’ll do:

  • Carry out supervisory responsibilities in accordance with North Suffolk’s policies and applicable laws including but not limited to the following:
    • Interview, select and hire employees.
    • Direct, assign, schedule and transfer employees.
    • Planning and training of employees.
    • Discipline and discharge employees for just cause.
    • Adjust employee grievances, problems and/or complaints.
    • Evaluate and appraise employee performance.
    • Make recommendations on the retention, compensation, promotion, layoff and/or recall of employees.
  • Manage a multidisciplinary treatment team in the following:
    • Maintain and encourage team’s adherence to professional and managed care standards.
    • Insure maintenance of consumer records in accordance with applicable regulatory (i.e. DMA, DPH, BSAS, Federal Probation) and NSMHA standards.
    • Manage timely and proper team treatment planning, action plan reviews, and periodic case reviews.
    • Identify necessity and coordinate Medical Peer Reviews.
    • Review and recommend staff requests for time off in consideration of insuring adequate coverage.
  • Provide direct clinical service.
    • Perform biopsychosocial, multiaxial assessment and diagnosis of potential consumers;
      • present findings of same to team;
    • Clinically administer caseload of consumers.
      • Coordinate the treatment team’s formulation and implementation of individualized consumer-centered action plans; monitor progress, reevaluate and revise on a weekly basis.
      • Oversee and participate in activities of service coordination, consultation and evaluation to successfully manage caseload within the team context.
      • Oversee and complete all clinical documentation and prior approval forms required.
    • Facilitate psychoeducational, psychotherapeutic, and or psychosocial rehabilitative groups with program consumers.
    • Coordinate and provide behavioral crisis intervention and or triage with program consumers, as needed.
  • Act as liaison between program and public.
    • Provide marketing support in the community as identified by Department Director.
    • Provide consultation and education to community agencies.
  • Coordinate program review, revision and development.

What you’ll need:

  • Education: A Master’s degree in an applicable field required.
  • Experience: Group supervision and facilitation skills, and experience with dually diagnosed SPMI population are required; length of experience contingent upon educational credentials.
  • Skills/Certifications: Full license/certification preferred or as required for core staffing complement.  Driver’s license and access to a dependable vehicle preferred.
  • Vaccination: At least one shot of the COVID-19 vaccination is required prior to hire and complete the full vaccination schedule in the appropriate timeline as documented by the CDC requirements for vaccination.
