Emergency Respite Clinical Coordinator
North Suffolk is looking for a Clinical Coordinator to join our Emergency Respite Program team! The Clinical Coordinator leads the team in the assessment, treatment planning and service delivery processes for individuals served by the Emergency Respite Program. The Clinical Coordinator oversees the development of the persons’ CSP (Community Support Plan) and subsequent reviews and revisions to ensure the goals, objectives and interventions reflect the Clinical Formulation-Interpretive Summary and the prioritized assessed needs. Services are provided based on Recovery tenets and are aimed to promote overall health and well-being of persons served.
Two positions available:
- Monday – Friday, 8am – 4pm
- Tuesday – Saturday, 8am – 4pm
What you’ll do:
- Serve as member of a multi-disciplinary team designed to help divert hospital admissions in a site-based setting and in various community location.
- Engage people and partner with them to improve their health and wellness, achieve symptom stabilization and self-management, develop and/or restore skills impacted by mental illness and live a self-directed life.
- Assist people with psychiatric and substance use disorders to identify personal priorities, preferences, strengths, and interests in order to help them establish goals that support a life in recovery.
- Instill hope by engaging in positive interactions that promote recovery and empowerment.
- Provide clinical services in timely and flexible ways that are consistent with the principles and practices of trauma-informed care and person-centered approaches.
- Provide practical help and supports, advocacy, coordination, side-by-side individualized support, problem solving, direct assistance, training, and supervision to help people obtain services and supports including medical and dental health care; legal and advocacy services; financial support such as entitlements and benefits, etc.
- Utilize the principles of psychiatric rehabilitation and recovery, lead the Emergency Respite Program’s efforts to provide the following to persons served: Engagement, Assessment, Skill building, Medication Administration, etc.
What you’ll need:
- Education: LPHA designation that is recognized under Massachusetts law is required.
- Experience: Previous experience in the provision of clinical or rehabilitative services to adults with chronic and persistent mental health issues and/or substance use disorders preferred. Previous supervisory experience preferred, but not required.
- Skills/Other: Must have Driver’s license. Must be MAP certified and CPR/1st aide certified within the first three months of hire.
- Vaccination: At least one shot of the COVID-19 vaccination is required prior to hire and complete the full vaccination schedule in the appropriate timeline as documented by the CDC requirements for vaccination.
