Community Coordinator – STEPRox
Are you active in your community? Familiar with substance abuse and mental health? Ready to use your skills and knowledge to give others a helping hand? NSMHA is looking for outgoing, friendly community coordinators to provide case management services to individuals with substance use disorders as a part of our STEPRox Recovery Support Center. These services consist of outreach activities, eligibility determination, service plan development, service coordination, individual counseling, group facilitation, resource development and client advocacy.
What you’ll do:
- In collaboration with the Program Manager and Director of Diversionary Addiction Services, participate in comprehensive assessment.
- Prepare and maintain client documentation including: MIS and ASI, treatment service plans, progress and follow-up contacts notes, and discharges.
- Select the appropriate service resources to support client’s recovery.
- Oversee peer activities.
- Provide and/or assist with transportation for clients to detox, rehab, and other support programs.
- Consult on cases with probation officers, DSS, attorneys, and other provider agencies.
- Advocate for clients in court, before the parole board and at other agency hearings.
- Effectively intervene and mediate crisis situations and circumstances.
- Provide individual counseling and facilitate support groups.
- Compose letters and other correspondence as requested by clients, courts and/or other agencies.
- Assist in organizing support and attend STEP’s funding activities, and act as a liaison for STEP with other service providers, criminal justice system and the community at large.
- Provide outreach services to neighborhood residents and the community at large.
- Assist in maintaining the Agency’s security and housekeeping activities, and perform any additional tasks that help support the smooth operation of the counseling/case management team and the agency at large.
- Coordinate the work schedules of the direct service staff.
- Prepare and coordinate the celebration activities including anniversary medallion presentations.
- Distribute mail to staff from incarcerated individuals seeking support.
- Ensure that on-site Fellowship meetings have appropriate coverage.
What you’ll need:
- Education: Bachelor’s Degree in Human or Social Services or a minimum of a high school diploma
- Experience: One year of supervised experience in substance abuse, a related human services field, or equivalent. Knowledge and understanding of: addiction as a disease, individual recovery counseling and group facilitation. Knowledge of metal health and substance abuse services
- Skills: A valid Massachusetts driver’s license and vehicle required
