NSMHA is looking for a Care Navigator to join our new CCBHC division. The Care Navigator coordinates systems navigation, participation and transition into a behavioral health/addictions outpatient treatment program. The Care Navigator educates and supports individuals during intake with accessing community resources, assists with intake documentation and requirements, and provides engagement outreach. The Care Navigator is a liaison with family, care providers, natural supports and recovery coaches.
Spanish Bilingual required. Please note language skills in application.
What you’ll do:
- Coordinate referrals; communicate fully and effectively with referral sources to facilitate prompt and comprehensive responsiveness.
- Coordinate communication and timely information about participants’ program adherence to referral sources and collateral entities such as the courts, Department of Children and Families, etc.
- Coordinate with collateral providers and community partners such as health care providers, outpatient behavioral health providers, recovery coaches, etc.
- Perform outreach and engagement activities with program participants, as needed.
- Under the supervision of a manager, identify risk and behavioral crisis needs and bring to the attention of clinical staff as appropriate and as needed.
- Meet individually with program participants as indicated to complete intake paperwork, and address administrative or collateral communication needs, etc.
- Complete all clinical documentation as required in the electronic health record or other databases.
- Maintain and update databases and spreadsheets, as directed, to assist with tracking clinical information, collateral communication, client assignments, etc.
- Follow and support individuals throughout intake until the process is completed, and as necessary during early treatment.
- Participate in staff meetings and agency meetings as required.
- In collaboration with clinical staff, develop, contribute to and Implement individual action plans, monitor and report progress and document as appropriate to credential level.
- Maintain professional, productive, respectful and cordial relationships with all fellow staff members, collateral providers, community partners and persons served.
- Perform other duties or tasks as assigned by Director or designee.
What you’ll need:
- Education: Bachelor’s degree in Social Work, Counseling Psychology, Psychology, Expressive Therapies, or related field of study.
- Experience: A minimum of 2 years post-degree experience within a multidisciplinary treatment setting. Group facilitation skills and experience with individuals with substance use disorders required; length of experience contingent upon educational credentials.
