The Intensive Care Coordinator (ICC) position is not only about coordination, it is also assessing and identifying the underlying needs of the youth through regular contact with the family in order to make a direct positive impact in their lives using the Wraparound Service Model. We support families to recognize small areas of progress that builds into major changes. For each family that you work with, you will not only be the key contact for the youth’s providers, you will also be creating an individualized roadmap that guides uniform treatment among the team. As the contact, you will collaborate with multi-disciplinary teams such as PCP, school, DCF, Outpatient therapists and other mental health and/or medical providers.

What You’ll Do:

  • Gather information for the completion of strengths based comprehensive home-based assessments.
  • Support with the identification and development of natural supports within the family’s network
  • Facilitate regular Care Planning Team meetings
  • Develop a youth and family centered Individual Care Plan (ICP) that supports the achievement of goals identified with the family’s voice and choice in conjunction with the care providers, natural supports and other identified team members.
  • Maintain regular contact with the youth, family and other providers in the youth’s life by phone, community meetings or home visits to ensure implementation of the ICP.
  • Provide education, advocacy and support to youth and families navigating through any of the child/family-serving systems
  • Identify and connect youth and families to additional supports and services in the community through referral and coordination of care
  • Develop a risk management/safety plan and provide as needed crisis support, as well as coordination with the Mobile Crisis Intervention team.

What You Need:

  • Education: Master’s degree or bachelor’s degree in relevant mental health field required (including but not restricted to, counseling, family therapy, social work, psychology, etc.)
  • Skills: Must have or be able to acquire Child and Adolescent Needs and Strengths (CANS) certification within first month of employment. Must have Driver’s License and reliable automobile. Ideal candidate must thrive in fast paced work environments; have strong attention to detail, and demonstrate strong organization and multitasking skills.

NSMHA is looking for a Family Partner to join our Community Service Agency!  This paraprofessional position provides a structured one-to-one, strength-based relationship to a parent/caregiver of a Seriously Emotionally Disturbed youth for the purpose of addressing the youth’s emotional and behavioral needs.  This is accomplished by enhancing parenting capacity and increasing access to natural community resources to enable the parent/caregiver to parent the youth successfully.

What you’ll do:

  • Engage the parent/caregiver in activities in the home and community.
  • Assist the parent/caregiver in addressing one or more goals on the youth’s treatment plan for Outpatient or In-Home Therapy or Individual Care Plan.
  • Coordinate Intensive Care Coordination Services with the Care Coordinator within the agency and Hub providers outside of the agency.
  • Assist the parent/caregiver in meeting the youth’s needs by educating, supporting, coaching, modeling or guiding and fosters empowerment.
  • Deliver services in the parent/caregiver’s home and community.
  • Participate, attend and support parent/caregiver and Care Coordinator’s for Individual Care Plan meetings.
  • Maintain regular contact with the parent/caregiver (face-to-face and telephone).
  • Provide family education, advocacy and support to youth and family.
  • Monitor, review and update the ICP to reflect the changing needs of the youth and family.
  • Adhere to policies and procedures governing client rights and confidentiality.
  • Complete appropriate documentation of service delivery in a timely manner.

What you’ll need:

  • Education
    • BA in a Human Services field and one year of experience OR
    • Associates degree in a human service field and one year of related experience OR
    • High School diploma/GED and a minimum of two years relevant experience.
  • Experience: 
    • Experience as a caregiver of a youth with special needs (preferably mental health needs).
    • Experience working with parent/caregivers and youth with Serious Emotional Disturbance.
    • Experience navigating any of the child and family serving systems.
    • Familiarity with child & youth serving programs
  • Skills:

North Suffolk is looking for a Community Navigator to join our new program, the Latino Community Action Response Team (Latino CART).  Latino CART provides clinical and case management support to uninsured/underinsured immigrant individuals and their families living in Chelsea, MA. In conjunction with the Latino CART Manager, the Latino CART Navigator provides intensive case management and therapeutic inventions to this unique underserved population to further support their stability within the Chelsea community. Case management and therapeutic inventions may include providing advocacy, outreach, assistance in connecting with basic needs, finding solutions to problematic behaviors, skill building and facilitating linkages with other community based and natural support systems.

What You’ll Do:

  • Assist qualifying Latino CART clients in navigating and connecting with community resources that will further support the person’s stability within their community. This includes connections with: Community food resources (SNAP, food banks), Housing resources (Boston Public Housing), Transportation (PT1, The RIDE), Legal services and/or others
  • Support Latino CART clients in meeting SMART goals outlined in treatment plan.
  • If indicated on treatment plan, support Latino CART Manager provide family intervention services to improve family dynamics among the client and their family members/natural supports

What You’ll Need:

  • Education/Experience: 
    • Bachelor’s degree in a human service field from an accredited university and one year experience working with children/adolescents/transition age youth  OR
    • Associate’s degree in a human services field from an accredited school and one year of experience working with the target population
