North Suffolk is looking for a Nurse Care Manager to join our Behavioral Health Community Partner (BH CP) team!  The Nurse Care Manager serves as a member of the BH CP team, providing direct care management and coordination to Enrollees.  The Nurse Care Manager collaborates with medical and other care providers and provides consultation to team members regarding health needs, health behaviors and medications of the team’s Enrollees; as well as participates in the ongoing development of trainings for staff and Enrollees regarding health and health behaviors.

Due to COVID, this position is currently primarily remote with some in-office time required.

We are currently offering a $2,000 sign on bonus for this position!

What you’ll do:

  • Provide all nursing and related aspects of care management and coordination services to the Enrollees of the BH CP, including but not limited to medication reconciliation, discharge planning, coordination with physicians, reviewing Enrollee Assessment health information, collaborating on Assessments and Care Planning, etc.
  • Consult with BH CP staff,  Program Director, Medical Director, and Care Team  members, PCP and/or MCO/ACO regarding Enrollees’ health related activities, interventions and service needs, as needed.
  • Communicate directly with Enrollees via telephonic, electronic and face to face modalities
  • Collaborate with program management and staff to develop and provide training for staff and Enrollees on health and wellness, and healthy behaviors.
  • Provide other direct services in keeping with the Nurse Care Manager role, including serving as primary care coordinator for some Enrollees, as may be assigned by the Clinical Care Manager/Team Leader.
  • On-call pager services as required.
  • Complete all clinical and administrative documentation in a thorough, efficient and timely way, in compliance with organization, payer, state and other applicable protocols.
  • Comply with all mandated reporting requirements and regulations (i.e. DCF, DMH, DPPC, Elder Protective Services).
  • On-call pager services as required.

What you’ll need:

  • Education/Certification:  Registered Nurse with current Massachusetts license to practice.  Bachelor’s degree in nursing preferred.
  • Experience:  Experience with people with mental illness and substance use disorders, with at least one year of providing care to individuals with medical problems preferred.  Embraces person-centered planning philosophy and motivational interviewing
  • Other requirements:   Valid Mass Drivers License and access to reliable vehicle required.

North Suffolk is looking for a Nurse Manager to join our team!  The Nurse Manager provides skilled nursing, care management and coordination for clients receiving services in the Addiction and Diversionary programs of NSMHA, including RRS, COE, IOP, Day Treatment and Outpatient. You’ll work in close collaboration with prescribing physicians, psychiatric nurse practitioners, behavioral health providers, recovery coaches, other care team members, and our referral partners in the community as well as make plans for the ongoing long term care of clients in recovery from substance abuse.

What you’ll do:

  • Provide skilled nursing, care management, medication management, group facilitation, and care coordination to clients in addiction and diversionary programs.
  • Conduct initial assessment and intake for clients seeking treatment for addiction, as well as ongoing education and support in all phases of treatment.
  • Provide linkage and referral to in-house and community-based behavioral health, case management and addiction recovery services as needed.
  • Collaborate in a multidisciplinary team to review evaluations, diagnosis, and treatment plans for clients as well as provide consultation to programs as needed.
  • Provide support to the medical team including health promotion, disease screening, data collection and wellness interventions in the programs.
  • Perform on-call duties as assigned during business hours.
  • Assist as needed in program development and staff training.
  • May participate as a member of the Addiction Division Medical Assistance Treatment (MAT) team.  Responsibilities include providing injectable medications as prescribed, ordering and reviewing labs in collaboration with prescribers.

What you’ll need:

  • Education:  Bachelors Degree in Nursing or related field required.
  • License:  Current Massachusetts Nursing License required.
  • Experience/Skills:  Two (2) years nursing experience required.  Prior case management, mental health and addiction experience strongly preferred.  Demonstrated ability to work with patients in recovery and relapse.  Bilingual Spanish preferred — please indicate language ability on resume!
