As a Licensed Practical Nurse for North Suffolk Mental health you will provide medical, regulatory (DDS regulations and MAP policy) oversight, support to residential program. The LPN is responsible for overseeing all health care services provided to individuals. Expectations of the Role:

  • Assist with health care services provided to individuals supported. Participate in medical discharge meetings at hospitals.
  • Assists to obtain reports and approval of treatment plans from PCP/specialists and reports and objectively documents all changes in the condition of each individual.
  • Reviews seizure and medical incident reports and makes recommendations to program staff and assist with any follow up with participant’s medical practitioners if necessary
  • Participate in managers’, clinical and staff meetings to review health care needs and global health care planning with staff.

NSMHA is looking for a Clinical Coordinator to join our Meridian House team!  The Clinical Coordinator reports to the Director of Addictions and works in collaboration with the House Manager and Clinical Manger of Ambulatory Services.  This position is responsible for the overall coordination of clinical matters including supervision, investigating client grievances, recommending programmatic changes, interfacing with other providers, utilization review of medical records, and providing direct services to clients.

Located in East Boston, the Meridian House is a 30 bed, coed therapeutic community serving an adult population 18 and over. Meridian House is considered to be a High Intensity Residential Service. What you’ll do:

  • Coordinate with the Intake Coordinator to approve appropriate clinical placement ensuring ASAM criteria is met upon admission.
  • Complete Adult Comprehensive Assessment and Individualized Action Plan upon admission.
  • Coordinate with prescribers, Manager of Rapid Access, Central Triage Team and Outpatient Clinical Team to ensure comprehensive and coordinated clinical services
  • Provide appropriate supervision and support to case managers and counselors
  • Coordinate and monitor care received by all residents to assure appropriate direct care, case management, continuity of care, and transitional services are provided.
  • Maintain quality of care according to standards and procedures outlined in DPH regulations and other contractual obligations.

What you’ll need:

North Suffolk is looking for a Recovery Specialists for our residential substance use program!  The Recovery Specialist is responsible for the health, safety and quality of life of individuals in a residential setting through advocacy, teaching and service planning, individual and group counseling. This is a 40 hour per week position with no weekend hours and no on-call requirement.

  • Guide and assist residents on meeting treatment plan goals
  • Oversee the milieu to ensure residents are safe and supported

North Suffolk is looking for a Program Coordinator to join our team!  The Program Coordinator is responsible for administrative operations of the Hanton House, our newest residential addictions program located in Chelsea, MA.  This position is responsible for the oversight of kitchen, food procurement, purchase ordering (paper goods, toiletries, linens, furniture, office supplies, etc.) and carries a small case management caseload.

What you’ll do:

  • Be responsible for all administrative duties associated with the Hanton House, including ensuring compliance with the BSAS and NSMHA policies and procedures.
  • Maintain a small case management caseload (2-4 residents)
  • Support the development and maintenance of the Hanton House by working closely with Program Manager, Clinical Coordinator and Intake Coordinator.
  • Facilitate staff meetings and non-clinical resident groups
  • Serve as culturally and linguistically appropriate services (CLAS) point person.
  • Assist management in resolving issues relating to the Hanton House program.
  • Conduct public relations activities, not limited to but including the Hanton House Steering Committee

  • Supervise the bachelors level staff, under the guidance of the Program Manager.

As a Certified Nursing Assistant for North Suffolk Mental Health Association, you’ll participate in the engagement, assessment and treatment planning process and as part of the program. Under the supervision of the Program Manager, the Overnight C.N.A is responsible for the safety and well-being of persons in GLE settings during overnight shifts. This position provides direct services to people in GLE settings based on need level and adheres to all agency policies and procedures and licensing/funding source requirements.

Expectations of the Role:

  • Be awake, and available to persons during overnight shift until persons have left residential program, or until the morning staff person arrives.
  • Check in with staff, persons who are awake and complete a walk-through of the program every hour. Read the Staff Communication Log and review risk plans.
  • Document each person’s progress in the confidential record according to established methods and procedures and carries out the assigned interventions and action steps of the treatment plan.
  • Perform household maintenance and upkeep as needed.
  • Participate in all required clinical and skill trainings to enhance skills and knowledge base.
  • Assist in showering if needed, laundry throughout the night, and cleanliness of the entire program.

As an Assistant Residential Program Manager, you will be responsible responsible for working directly with individuals and staff to ensure that the needs of the Individuals living in the assigned group home(s) are met in all areas of service.
