What you’ll do:

  • Recruit volunteers and create linkages.
  • Perform the initial screening process for all volunteers.
  • Facilitate the orientation of peer members and volunteers and establish a schedule of participation.
  • Assist in the training of peers to become facilitators of peer groups.
  • Meet regularly with volunteers to update them about opportunities and resources.
  • Develop and maintain a up-to-date volunteer database and tracking system.

What you’ll need:

As a Community Support Specialist you will participate in the engagement, assessment, and treatment planning process as well as work with a dedicated team of staff to provide interventions to address needs and goals of the persons served. This position provides direct services to people in community and group living environment settings in the areas of housing, risk mitigation, transportation, skill-building, money management, medication administration and other areas based on the person centered treatment plan.
Services are provided based on Recovery tenets and are aimed to promote overall health and well-being of persons served.

Expectations of the Role:
• Engage people and partner with them to improve their health and wellness, achieve symptom stabilization and self-management, develop and/or restore skills impacted by mental illness and live a self-directed life.

• Organize and lead individual and group social and recreational activities.
• Carries out the assigned interventions and action steps of the treatment plan, through the provision of direct support and rehabilitation services to people on an individual, group, and family basis (in conjunction with professional staff) in the office and in community settings.
• Experience in coordinating collaborative care within teams
• Collaborate with Housing Coordinators to help each person in finding a safe and affordable place to live – apartment hunting, finding a roommate, landlord negotiations, cleaning, furnishing and decorating
• Ensure that people have adequate financial support
• Participate in medication administration per MAP and DMH policies and protocol.
• Help each person to access reliable transportation

What you’ll need:
Education: High School Diploma/GED equivalent required. Bachelors Degree highly preferred
Experience: In lieu of college degree, at least one year experience with population that has severe and persistent mental illness and/or substance use disorder
Skills: Must be MAP and CPR/First Aid certified within five months of hire (training provided) Massachusetts Drivers License required

  • Establish and maintain relationships with clients, providing contact and support.

  • Support, coach and train youth in order to support linkages to community resources and services that will sustain the youth’s optimal functioning in the community.
  • Work closely with the family, and any behavioral health existing/referring provider(s) to implement the goals and objectives identified by the referring provider, when the youth is not enrolled in Intensive Care Coordination (ICC).
  • Participate as a member of the Care Planning Team (CPT) and clearly outlines the goals of the Therapeutic Mentoring Service in the Individual Care Plan (ICP), when the youth is enrolled in Intensive Care Coordination (ICC).
  • Assist youth in communicating their needs to the treatment team when age-appropriate.
  • Contributes the voice of the youth in the absence of the youth.
  • Work in the community and in-home settings.

NSMHA is looking for a Care Navigator to join our new CCBHC division.  The Care Navigator coordinates systems navigation, participation and transition into a behavioral health/addictions outpatient treatment program.  The Care Navigator educates and supports individuals during intake with accessing community resources, assists with intake documentation and requirements, and provides engagement outreach.  The Care Navigator is a liaison with family, care providers, natural supports and recovery coaches.

What you’ll do:

  • Coordinate referrals; communicate fully and effectively with referral sources to facilitate prompt and comprehensive responsiveness.
  • Coordinate communication and timely information about participants’ program adherence to referral sources and collateral entities such as the courts, Department of Children and Families, etc.
  • Coordinate with collateral providers and community partners such as health care providers, outpatient behavioral health providers, recovery coaches, etc.
  • Perform outreach and engagement activities with program participants, as needed.
  • Under the supervision of a manager, identify risk and behavioral crisis needs and bring to the attention of clinical staff as appropriate and as needed.
  • Meet individually with program participants as indicated to complete intake paperwork, and address administrative or collateral communication needs, etc.
  • Complete all clinical documentation as required in the electronic health record or other databases.
  • Maintain and update databases and spreadsheets, as directed, to assist with tracking clinical information, collateral communication, client assignments, etc.
  • Follow and support individuals throughout intake until the process is completed, and as necessary during early treatment.
  • Participate in staff meetings and agency meetings as required.
  • In collaboration with clinical staff, develop, contribute to and Implement individual action plans, monitor and report progress and document as appropriate to credential level.
  • Maintain professional, productive, respectful and cordial relationships with all fellow staff members, collateral providers, community partners and persons served.

  • Assess crisis or emergency situations and intervene immediately with appropriate clinical treatment for clients.
  • This is an on-site position, with the expectation of mobile response, in addition.
  • Weekends, Holiday and shift rotation will be required.

  • Experience: Minimum one year in a psychiatric emergency service or similar setting required.
  • Education: Master’s Degree in a relevant human service clinical specialty that is Medicaid reimbursable.
