NSMHA is looking for a Family Partner to join our Community Service Agency!  As a Family Partner, you will provide a structured one-to-one, strength-based relationship to a parent/caregiver of a Seriously Emotionally Disturbed youth for the purpose of addressing the youth’s emotional and behavioral needs.  You will work with parents and caregivers to enhance parenting capacity and increase access to natural community resources to enable the parents/caregiver to parent the youth successfully.

This position is designed for someone with lived experience as a parent or direct caregiver of a youth in their family with special needs (preferably mental health needs).

What you’ll do:

  • Engage the parent/caregiver in activities in the home and community.
  • Assist the parent/caregiver in addressing one or more goals on the youth’s treatment plan for Outpatient or In-Home Therapy or Individual Care Plan.
  • Coordinate Intensive Care Coordination Services with the Care Coordinator within the agency and Hub providers outside of the agency.

What you’ll do:

  • Assist with supervisory responsibilities in accordance with North Suffolk’s policies and applicable laws

Work closely with the Boston Medical Center (BMC) Call Center on the assignment of cases and additional triaging, when necessary.
  • Provide in-the-moment supervision to mobile clinicians and other staff and provide training to newer staff, while also conducting face to face encounters in various mobile locations
  • Manage all aspects of open and ongoing cases, including boarded patients including
    • placement, insurance authorizations, and close clinical management of boarded patients,
    • following ESP guidelines of placement in the most appropriate, least restrictive placement settings.
  • Monitor productivity, caseloads and units-of-work level of clinicians and family partners
  • Perform clinical services, crisis intervention and appropriate evaluations as needed.
  • Develop and monitor all staff, program, and facility policies.

NSMHA is looking for a Grand Manager to join our Winthrop MINDeaze team!  The Grant Manager works in close collaboration with Winthrop Public Schools to provide clinical and coordination services to vulnerable 8-13 year old students and their families within a school based setting. They identify student’s needs through clinical assessment and provide appropriate evidence based treatment to address student’s needs. Interventions are designed to be intensive and immediate to improve the student’s ability to function as a unit both in the school and in the community. The Grant Manager for Winthrop Public School will be trained in evidence based practices and tools to be used to promote positive social activity, positive peer relationships, improved relationship with family, and improved recovery environment and abstinence from illicit drugs and alcohol.

What you’ll do:

  • Outreach and engage students and their families referred by school administration to participate in therapeutic and clinical services embedded within Winthrop Public schools.
  • Complete a written comprehensive psychosocial behavioral health assessment for students identify on caseload.
  • Develop a treatment plan sensitive to the student and their family history in collaboration with the youth, parent, guardian, or designated caregiver(s), and Winthrop Public School with required consent and in consultation with other providers.

We support families to recognize small areas of progress that builds into major changes. For each family that you work with, you will not only be the key contact for the youth’s providers, you will also be creating an individualized roadmap that guides uniform treatment among the team. As the contact, you will collaborate with multi-disciplinary teams such as PCP, school, DCF, Outpatient therapists and other mental health and/or medical providers.

This position is currently a hybrid position: community visits and remote work, contingent on family preference. 

What You’ll Do:

  • Gather information for the completion of strengths based comprehensive home-based assessments.
  • Support with the identification and development of natural supports within the family’s network
  • Facilitate regular Care Planning Team meetings
  • Develop a youth and family centered Individual Care Plan (ICP) that supports the achievement of goals identified with the family’s voice and choice in conjunction with the care providers, natural supports and other identified team members.
  • Maintain regular contact with the youth, family and other providers in the youth’s life by phone, community meetings or home visits to ensure implementation of the ICP.
  • Provide education, advocacy and support to youth and families navigating through any of the child/family-serving systems
  • Identify and connect youth and families to additional supports and services in the community through referral and coordination of care
  • Develop a risk management/safety plan and provide as needed crisis support, as well as coordination with the Mobile Crisis Intervention team.

What You Need:

  • Education: Bachelor’s degree in relevant mental health field required (including but not restricted to, counseling, family therapy, social work, psychology, etc.) Masters degree preferred.
  • Skills: Must have or be able to acquire Child and Adolescent Needs and Strengths (CANS) certification within first month of employment. Must have Driver’s License and reliable automobile. Ideal candidate must thrive in fast paced work environments; have strong attention to detail, and demonstrate strong organization and multitasking skills.
  • Experience: Familiarity with child & youth serving programs preferred.  Bilingual candidates strongly encouraged to apply! We offer a 12% language differential pay! Please note language skills on your resume.
  • Vaccination: At least one shot of the COVID-19 vaccination is required prior to hire and complete the full vaccination schedule in the appropriate timeline as documented by the CDC requirements for vaccination.
