As an Addictions Specialist, you’ll lead the team in working with persons served to understand how substance use and potentially addictive behavior impacts the person’s health.  Your goal will be to help the team develop a plan to address the level of use and the impact of substance use and addictive behavior and the persons’ readiness for change.

The Adult Community Clinical Services Program (ACCS) assists more than 450 adults diagnosed with serious and persistent mental illness who have been referred by the Department of Mental Health. This is a clinical program based on the principles of Psychiatric Rehabilitation, person-centered planning, and the belief that recovery is possible. Persons served may live on their own, in group homes or in North Suffolk owned properties. Each person served works with an Integrated Team that includes licensed clinicians, psychiatrists, nurses, addiction specialists, peer specialists and community support specialists.

We are offering a $2,000 sign on bonus for this position.

Expectations of the Role:

  • Works collaboratively with Recovery Coaches to provide teaching and modeling to the teams on addressing substance use concerns.
  • Provides direct interventions and supports to persons.
  • Refers persons to community treatment and support programs.
  • Provides, as needed, individual, group, and family counseling and psychoeducational services regarding addictions.
  • Is trained in evidence based practices including SBIRT, Motivational Interviewing, Stages of Change model and Harm Reduction.
  • May provide functions of LPHA in the Integrated Team if person possesses LADC-1 licensure level.
  • Document each person’s progress in the confidential record according to established methods and procedures.
  • Carries out the assigned interventions and action steps of the treatment plan, to people on an individual, group, and family basis in the office and in community settings.
  • Teach behavioral symptom-management techniques to alleviate and manage symptoms not reduced by medication and to promote personal growth and development by assisting people to adapt to and cope with internal and external stresses.
  • Provide interventions to assigned persons in GLE settings

Education:  High School Diploma/GED equivalent required.  Bachelors Degree highly preferred

As a Peer Specialist for North Suffolk Mental Health, you’ll build supportive relationships that promote empowerment and self-learning. The Peer Support Specialist position is designated for a person who has lived experience and wants to help guide others through the recovery process.

The ACCS Program uses clinical and evidence based practices to uncover motivation while working with people to establish meaningful treatment plans and cultivate recovery. Each individual in the program is assigned a primary clinician and works with an integrated team composed of licensed clinicians, peer/recovery specialists and others to meet their individual needs.  This position requires excellent communication, writing, and organizational skills and the ability to work independently in the community. Extensive training is provided and ongoing training is available and encouraged.

Expectations of the Role:

  • Support persons served through Life and Care transitions, including visiting a hospital or other restrictive setting in order to provide connection and transition planning.

Our BH CP program has an opening for a Care Coordinator to support the health, safety and quality of life of clients through advocacy, teaching, and service planning. In this role, you will provide person-centered and recovery oriented care management and coordination. Care management may include but not be limited to outreach and engagement, assisting Enrollees to identify goals and to develop an individual care plan, completing comprehensive assessments, facilitating Care Teams, and coordinating communication among Enrollees, their healthcare and other providers, involved state agencies and others, and supporting Enrollees to access and utilize resources to improve and maintain their health and quality of life.

  • Conduct outreach and engagement activities to encourage enrollment.

The Behavior Therapist Outreach Worker is a member of the In Home Support Team. This position provides direct services to people in the community with a variety of diagnoses, including Autism Spectrum Disorder (ASD).  This position will provide support in the areas of housing, risk mitigation, transportation, skill-building, employment, money management, community access, medication administration, scheduling, behavior management, Activities of Daily Living (ADLs) and other areas based on the Individual Support Plan.  The Behavior Therapist works with the support of a Board Certified Behavior Analyst (BCBA) and has experience implementing the principles of Applied Behavior Analysis (ABA).

Schedule: Monday-Friday 10am-6pm

The Outreach Worker provides intensive case management services to adults and children with mental health and substance abuse issues who are considered to be at risk in the community. This position coordinates the case navigation, participation and transition within a behavioral health/addictions treatment services structured intensive outpatient substance use program.  The Care Coordinator educates and supports individuals diagnosed with substance use disorder and co-occurring disorders in their transition to living sustained recovery through the use of behavior modification and concrete support with accessing community resources.  The Care Coordinator is a community liaison with family, care providers, natural supports and recovery coaches.

What You’ll Do:

  • Coordinate referrals; communicate fully and effectively with referral sources.
  • Coordinate communication and timely information about participants’ program adherence to referral sources and collateral entities such as the courts, Department of Children and Families, etc.
  • Coordinate with collateral providers and community partners such as health care providers, outpatient behavioral health providers, recovery coaches, etc.
  • Track and monitor urine toxicology screens for all clients, alerting clinicians as appropriate to results, and maintaining any related databases.
  • Facilitate treatment groups with program participants under the supervision of the program manager.
  • Perform outreach and engagement activities with program participants, as needed.
  • Under the supervision of the manager, identify risk and behavioral crisis needs and bring to the attention of clinical staff as appropriate.
  • Meet individually with program participants as indicated to review treatment plans, address administrative or collateral communication needs, etc.
  • Maintain and update databases and spreadsheets to assist with tracking clinical information
