North Suffolk is looking for a Family Community Coordinator to join our Chelsea Youth Access Center! It is a part-time position of 20 hrs per week. The Family Community Coordinator provides one-to-one support to Transitional Age Youth (TAY) within the Chelsea YA Center who is a Parent and or Caregiver. They deliver peer-to-peer education, coaching, modeling, and guidance to TAY Parent/Caregivers for the purpose of connecting to community resources that will further support and strengthen the TAY Parent/Caregivers’ parenting skills and stabilization within their community.

What You’ll Do:

  • Responsible for providing and/or connecting a TAY Parent/Caregiver with resources to further support the TAY’s parenting skills. This includes but is not limited to:
    • Education on how to building stronger empathic bond with your child
    • Psychoeducation on how to developed effective parental disciplining skills
    • Understanding on how to balance life goals for themselves in relationship to their child’s goals
  • Responsible for coaching a TAY Parent/Caregiver on how to navigate and connect with community resources framed at strengthen the TAY’s stability within the community. These community resources include but are not limited to:
    • Applying for benefit and entitlement programs
    • Employment and vocational training programs
    • Education services
    • Legal services
    • Housing services
  • Responsible for assisting the Chelsea YA Center staff in the delivery of the daily/weekly programming.
  • Facilitate parent support groups, as needed.
  • Provide support to TAY Parent/Caregivers during their child’s treatment team meetings, as needed.
  • Shares lived experience with TAY Parent/Caregivers to build rapport, engage and inspire the TAY Parent/Caregiver towards improving overall goal achievement.
  • Complete appropriate documentation of service delivery as outlined by Program Manager for the Chelsea YA Center
  • Provide marketing support in the community as identified by Program Manager for the Chelsea YA Center.

What you’ll need:

  • Education: Associate Degree in a Human Services field OR High School diploma/GED and one year of experience with target population.
  • Experience: Experience navigating community resources (i.e. benefit/entitlement programs, employment /vocational programs, etc.) Must have experience as a family member of a youth/young adult experiencing mental health.
  • Skills: Bilingual (Spanish) ability strongly preferred.
