High School Diploma or GED required. Bachelor’s Degree in a related field preferred. Two (2) years experience in human services and supervisory experience required. Previous experience with provision and/or management of psychiatric rehabilitation services preferred. Must have Driver’s license. The Family Partner reports to the CSA Program Director through the Senior Family Partner/Team Leader.  This paraprofessional position provides a structured one-to-one, strength-based relationship to a parent/caregiver of a Seriously Emotionally Disturbed youth for the purpose of addressing the youth’s emotional and behavioral needs.  This is accomplished by enhancing parenting capacity and increasing access to natural community resources to enable the parent/caregiver to parent the youth successfully.

Duties:

  1. Engages the parent/caregiver in activities in the home and community.
  2. Assists the parent/caregiver in addressing one or more goals on the youth’s treatment plan for Outpatient or In-Home Therapy or Individual Care Plan.
  3. Coordinates Intensive Care Coordination Services with the Care Coordinator within the agency and Hub providers outside of the agency.
  4. Assists the parent/caregiver in meeting the youth’s needs by educating, supporting, coaching, modeling or guiding and fosters empowerment..
  5. Delivers services in the parent/caregiver’s home and community.
  6. Participates, attends and supports parent/caregiver and Care Coordinator’s for Individual Care Plan meetings.
  7. Maintain regular contact with the parent/caregiver (face-to-face and telephone).
  8. Provide family education, advocacy and support to youth and family.
  9. Monitoring, reviewing and updating the ICP to reflect the changing needs of the youth and family.
  10. Adhere to policies and procedures governing client rights and confidentiality.
  11. Complete appropriate documentation of service delivery in a timely manner.

 

Requirements:

  • BA in a Human Services field and one year of experience or Associates degree in a human service field and one year of related experience or High School diploma or GED and a minimum of two years relevant experience.
  • Experience as a caregiver of a youth with special needs (preferably mental health needs).
  • Experience working with parent/caregivers and youth with Serious Emotional Disturbance.
  • Experience navigating any of the child and family serving systems.
  • Familiarity with child & youth serving programs
  • Bilingual in Spanish (Highly preferred)

JOB SUMMARY:

This member of the Medical Services Team (MST) multidisciplinary team is responsible for serving as the primary psychiatrist for individuals identified as needing MST services in the community; to provide psychiatric and medical assessment and treatment; clinical supervision, education and training of the team; and development, implementation, maintenance, and supervision of medication and psychiatric and medical treatment policies and procedures for this service.

Essential Duties and Responsibilities:

  • Provide psychiatric treatment for individuals identified as needing MST services.
  • Prescribe medications and conduct regular assessments of therapeutic responses and side effects and educate clients and paraprofessional staff regarding medication treatment regimes.
  • Participate in person served-centered comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors); physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships.
  • Collaborate with the MST nurses in assessment of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinating medical treatments with psychiatric treatment.
  • Collaborate with the MST lead nurse and staff in other NSMHA community based programs to develop, revise, maintain, and supervise team psychopharmacologic, medical treatment and medication policies and procedures.
  • With other team members, provide support, education, and counseling to persons served and family members to help them become knowledgeable about mental illness, collaborate in the treatment planning process when invited by person served, and assist in his/her family member’s progress.
  • Collaborate with all members of MST to provide crisis assessment and management and to develop crisis/safety plans.
  • Be available on-call to MST staff, as needed, for management of psychiatric crises requiring immediate intervention.
  • Participate in the assessment of need for guardianship and, when indicated, complete paperwork required to pursue guardianship for people served.
  • Participate in the assessment of need for representative payee-ship, and when indicated, complete paperwork that requires physician documentation.
  • Attend staff organizational meetings and treatment planning review meetings.
  • Train staff in the knowledge and skills basic to the treatment of severe and persistent mental illnesses.
  • Provide clinical supervision to professional staff.

POSITION REQUIREMENTS:

The psychiatrist is board certified or board eligible in psychiatry and licensed to practice medicine in the state of Massachusetts, and has strong clinical skills and experience providing treatment to persons with severe and persistent mental illnesses. Clinical supervisory experience is desirable.  The person in this position must have a clear understanding of the characteristics and problems of adults with severe mental illnesses and be committed to community service delivery.  Must be CPR certified within five months of hire.

The ACRA/ACC Clinician and Care Coordinator works as part of the Teen TEAM program.  The Teen TEAM program provides substance abuse, community-based treatment to at-risk, underserved adolescents and young adults who have co-occurring substance abuse and mental health diagnosis. Treatment and interventions are in conjunction with the family, care providers, natural supports and case managers from child serving programs, and the ACRA/ACC Clinician is responsible for developing treatment plans and risk management/safety plans supporting the achievement of goals identified by the client and caregivers.  Interventions are designed to be intensive and immediate (after placement or incarceration) to improve the family’s ability to function as a unit both in the home and in the community. The Clinician/Care Coordinator will be trained in CANS, GAIN, and A-CRA/ACC evidence based practices and tools to be used to promote positive social activity, positive peer relationships, improved relationship with family, and improved recovery environment and abstinence from illicit drugs and alcohol.

Master’s degree required.  Experience with substance abuse, LGBT youth, and wrap-around care models preferred.  Must have valid drivers license and be comfortable working in the community. Bilingual Spanish preferred.

JOB SUMMARY:

This position reports to a North Suffolk psychiatrist for clinical supervision and to the CBFS director or designee for administrative supervision.  This member of the Medical Services Team (MST) is responsible for providing leadership and a broad range of clinical services including psychopharmacology, counseling, psycho-education and medical/clinical management.

Essential Duties and Responsibilities:

  • Provide nursing interventions for individuals identified as needing MST nursing services.
  • Prescribe medications and conduct regular assessments of therapeutic responses and side effects and educate clients regarding medications.
  • Provide administration of oral and intramuscular medication.
  • Participate in person centered comprehensive assessment (including onset, course and effect of illness, past treatment and responses, risk behaviors; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships.
  • Prepare documentation for persons-served in a timely manner and in compliance with applicable laws, regulations and Agency standards.
  • Collaborate with the MST Nurses regarding assessment of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinating medical treatments with psychiatric treatment.
  • Collaborate with the MST Nurse and staff in NSMHA community based programs to develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures.
  • With other team members, provide support, education, and counseling to family members of clients to help them become knowledgeable about mental illness, collaborate in the treatment planning process, and assist in their family member’s progress.
  • Collaborate with all members of MST to provide crisis assessment and management.
  • Will be available on-call to MST staff, as needed, for management of psychiatric crises requiring immediate intervention.
  • Will participate in the assessment of need for guardianship and, when indicated, complete paperwork required for pursuing guardianships for people served.
  • Attend staff organizational meetings and treatment planning review meetings.
  • Train staff in the knowledge and skills basic to the treatment of severe and persistent mental illnesses.

 

POSITION REQUIREMENTS:

Master’s Degree in Nursing is required. MA license is required. ANA Certification as a Clinical Nurse Specialist or Nurse Practitioner is required. Experience in mental health setting is required, community mental health preferred. Bilingual in Spanish/English strongly preferred.
