The Awake Overnight Counselor is responsible for the safety and well-being of persons in a residential setting (MH or DS) during overnight shifts. Provide assistance to persons based on need level of persons, and adheres to all agency policies and procedures and licensing/funding source requirements.  The Awake Overnight Counselor is awake and available to consumers during the entire overnight shift.  Performs household maintenance and upkeep activities as assigned by supervisor. Performs security checks and document, on an hourly basis to ensure safety of consumers and facility.  Performs basic data collection, data entry, and tallying of information with respect to IAP goals and objectives, and protocol.

Counselors may be hired to work at either Mental Health or Developmental Disability residential programs

 

Job Requirements:

  • High school degree or GED required, Bachelor’s in related field preferred
  • One (1) year human services experience with similar population required
  • Must be or have the ability to become medication, SFA and CPR certified after training provided

Weekend Floaters are part-time (16 hours per week) positions, responsible for providing weekend coverage in a residential home setting; in either a Direct Support Professional or Awake Overnight capacity. Floaters may be hired to work at either Mental Health or Developmental Disability residential programs. Access to reliable transportation is required, to ensure the floater’s ability to travel to different sites quickly and efficiently.

Job Duties:

  • Identify and utilize community resources; develop and maintain natural supports for the youth and parent/caregiver(s) in order to promote sustainability of treatment gains.
  • Reviews and develops risk management plans, and implements focused interventions and behavioral techniques in collaboration with the youth, parent, guardian, and designated caregivers.
  • Completes a written comprehensive home-based, psychosocial behavioral health assessment in the youth’s home or other location of the family’s choosing.
  • Monitoring progress and attainment of treatment plan goals and objectives.
  • Work with the entire family, or subset of the family, to implement focused interventions and behavioral techniques to: enhance problem-solving, limit-setting, risk management/safety planning, communication, build skills to strengthen the family, advance therapeutic goals, or improve ineffective patterns of interaction.

 

Position requirements:

  • Master’s degree in a relevant field required (Family Therapy, psychology, social work or similar preferred).
  • MA License (LCSW, LICSW, LMHC) preferred.
  • Experience in providing home and community based intervention services preferred.
  • Certification in the Massachusetts version of the CANS.
  • Bilingual ability preferred, Spanish ability highly preferred. Must have valid drivers license, reliable transportation, and be comfortable working in the community.

The ACRA/ACC Clinician and Care Coordinator works as part of the Teen TEAM program.  The Teen TEAM program provides substance abuse, community-based treatment to at-risk, underserved adolescents and young adults who have co-occurring substance abuse and mental health diagnosis. Treatment and interventions are in conjunction with the family, care providers, natural supports and case managers from child serving programs, and the ACRA/ACC Clinician is responsible for developing treatment plans and risk management/safety plans supporting the achievement of goals identified by the client and caregivers.  Interventions are designed to be intensive and immediate (after placement or incarceration) to improve the family’s ability to function as a unit both in the home and in the community. The Clinician/Care Coordinator will be trained in CANS, GAIN, and A-CRA/ACC evidence based practices and tools to be used to promote positive social activity, positive peer relationships, improved relationship with family, and improved recovery environment and abstinence from illicit drugs and alcohol.

Master’s degree required.  Experience with substance abuse, LGBT youth, and wrap-around care models preferred.

JOB SUMMARY:

This position reports to a North Suffolk psychiatrist for clinical supervision and to the CBFS director or designee for administrative supervision.  This member of the Medical Services Team (MST) is responsible for providing leadership and a broad range of clinical services including psychopharmacology, counseling, psycho-education and medical/clinical management.

Essential Duties and Responsibilities:

  • Provide nursing interventions for individuals identified as needing MST nursing services.
  • Prescribe medications and conduct regular assessments of therapeutic responses and side effects and educate clients regarding medications.
  • Provide administration of oral and intramuscular medication.
  • Participate in person centered comprehensive assessment (including onset, course and effect of illness, past treatment and responses, risk behaviors; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships.
  • Prepare documentation for persons-served in a timely manner and in compliance with applicable laws, regulations and Agency standards.
  • Collaborate with the MST Nurses regarding assessment of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinating medical treatments with psychiatric treatment.
  • Collaborate with the MST Nurse and staff in NSMHA community based programs to develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures.

POSITION REQUIREMENTS:

Master’s Degree in Nursing is required. MA license is required. ANA Certification as a Clinical Nurse Specialist or Nurse Practitioner is required. Experience in mental health setting is required, community mental health preferred. Bilingual in Spanish/English strongly preferred.

JOB SUMMARY:

This member of the Medical Services Team (MST) multidisciplinary team is responsible for serving as the primary psychiatrist for individuals identified as needing MST services in the community; to provide psychiatric and medical assessment and treatment; clinical supervision, education and training of the team; and development, implementation, maintenance, and supervision of medication and psychiatric and medical treatment policies and procedures for this service.

Essential Duties and Responsibilities:

  • Provide psychiatric treatment for individuals identified as needing MST services.
  • Prescribe medications and conduct regular assessments of therapeutic responses and side effects and educate clients and paraprofessional staff regarding medication treatment regimes.
  • Participate in person served-centered comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors); physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships.
  • Collaborate with the MST nurses in assessment of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinating medical treatments with psychiatric treatment.
  • Collaborate with the MST lead nurse and staff in other NSMHA community based programs to develop, revise, maintain, and supervise team psychopharmacologic, medical treatment and medication policies and procedures.

  • Provide clinical supervision to professional staff.

POSITION REQUIREMENTS:

  • Board certified or board eligible in psychiatry and licensed to practice medicine in the state of Massachusetts
  • Has strong clinical skills and experience providing treatment to persons with severe and persistent mental illnesses.
  • Clinical supervisory experience is desirable.
  • The person in this position must have a clear understanding of the characteristics and problems of adults with severe mental illnesses and be committed to community service delivery.
  • Must be CPR certified within five months of hire.

This position is responsible for the management of homes where adults with mental health reside when a Program Coordinator position(s) is vacant or additional program oversight is necessary.  Responsible for ensuring the needs of the Individuals living in the assigned group home(s) are met in all areas of service.  Responsible for ensuring basic systems are in place and operating successfully within assigned residential sites  including but not limited to medication administration, shopping, menu/meal planning and the maintenance and safety  This position interfaces with staff, peers, senior management, families, neighbors, medical practitioners and CBFS teams.

 

POSITION REQUIREMENTS:

High School Diploma or GED required.

Bachelor’s Degree in a related field preferred

Six (6) years experience in human services and supervisory experience required

Previous experience with provision and/or management of psychiatric rehabilitation services preferred.

Must have Driver’s license

Must be MAP/CPR/Frist Aid certified within three (3) months of hire

Knowledge of DMH regulations needed

Knowledge of managing a group home
