JOB SUMMARY:

This position reports to a North Suffolk psychiatrist for clinical supervision and to the CBFS director or designee for administrative supervision.  This member of the Medical Services Team (MST) is responsible for providing leadership and a broad range of clinical services including psychopharmacology, counseling, psycho-education and medical/clinical management.

Essential Duties and Responsibilities:

  • Provide nursing interventions for individuals identified as needing MST nursing services.
  • Prescribe medications and conduct regular assessments of therapeutic responses and side effects and educate clients regarding medications.
  • Provide administration of oral and intramuscular medication.
  • Participate in person centered comprehensive assessment (including onset, course and effect of illness, past treatment and responses, risk behaviors; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living; and family structure and relationships.
  • Prepare documentation for persons-served in a timely manner and in compliance with applicable laws, regulations and Agency standards.
  • Collaborate with the MST Nurses regarding assessment of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinating medical treatments with psychiatric treatment.
  • Collaborate with the MST Nurse and staff in NSMHA community based programs to develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures.
  • With other team members, provide support, education, and counseling to family members of clients to help them become knowledgeable about mental illness, collaborate in the treatment planning process, and assist in their family member’s progress.
  • Collaborate with all members of MST to provide crisis assessment and management.

POSITION REQUIREMENTS:

The psychiatrist is board certified or board eligible in psychiatry and licensed to practice medicine in the state of Massachusetts, and has strong clinical skills and experience providing treatment to persons with severe and persistent mental illnesses. Clinical supervisory experience is desirable.  The person in this position must have a clear understanding of the characteristics and problems of adults with severe mental illnesses and be committed to community service delivery.  Must be CPR certified within five months of hire.

Job Summary:

Reports to the Director of Child and Family, responsible for ensuring the overall coordination of care for clients when IHT is the ranking Hub provider. Ensures that all services are provided in accordance with the IHT Performance Specifications and Systems of Care Philosophy.  This position is expected to provide program and clinical leadership.  The Program Manager works closely with NSMHA staff, parents/guardians, community providers and referral sources.  Responsible for delivery of administrative and clinical services of a multidisciplinary team.  Monitors the care provided by the team. Coordinates resources across programs. Provides clinical and administrative supervision to professional and paraprofessional staff as necessary. Ensures positive working relationship with payors.

 

Essential Duties and Responsibilities:

  1. Carry out supervisory responsibilities for clinicians, outreach workers, therapeutic mentors, and interns in accordance with North Suffolk policies and procedures, applicable laws and to meet the IHT Performance Specifications’ requirements including but not limited to the following:
  • Interview, select and hire employees
  • Direct, assign, schedule and transfer employees
  • Planning and training of employees
  • Discipline and discharge employees for just cause
  • Adjust employee grievances, problems and/or complaints
  • Evaluate and appraise employee performance
  • Make recommendations on the retention, compensation, promotion, layoff and/or recall of employees.

  2. Ensure that IHT staff collaborates with CBHI and other providers.

  10. Responsible for maintaining professional social work standards of the team.
  11. Manages all staffing issues including hiring, corrective action, performance evaluations and termination.
  12. Responsible for ensuring all staff are trained in clinical and administrative procedures.
  13. Monitors staff caseloads, service provision, and productivity to ensure quality of service delivery.
  14. Establishes and maintains relationships with collateral agencies serving the client population.
  15. Participates in and ensures that the IHT program is covered clinically and accessible for 24 hours a day, 7 days a week.
  16. Provides after hours on-call coverage.
  17. Assists in developing, monitoring and managing budgets, contracts, and third party billing.
  18. Assists in developing new contracts and marketing services to the community.
  19. May carry a caseload providing direct services and related responsibilities as needed.
  20. Coordinates client services across NSMHA programs.
  21. Manages Payor accounts, ensuring positive working relationships with all Payors

Minimum, LICSW, LMHC or PhD in psychology, must be licensed, third party reimbursable clinician status necessary. Three (3) to five (5) years clinical and administrative experience. Advanced clinical experience, and supervisory experience required. CANS certified.

A portion of this position will be providing direct behavioral health services to consumers, and to supervise the delivery of the same by other clinicians as an integral part of the program’s multidisciplinary team.

  1. Under the supervision of the Director of Community Based Services carry out supervisory responsibilities in accordance with North Suffolk’s policies and applicable laws including but not limited to:

    4.  Facilitate psychoeducational, psychotherapeutic, and or psychosocial rehabilitative groups with program consumers and families.

    1. Provide marketing support in the community as identified by Department Director.
    2. Provide consultation and education to community agencies.
  6. Assist the Department Director with the hiring of staff.

  9. Establish and maintain professional contacts with all vendors care and insurance carriers to ensure proper payment of all claims.
  10. Maintain respectful and cordial relationships with all collateral providers; identify marketing opportunities and provide marketing support as needed.
  11. Develop and maintain contacts with community agencies in support of client treatment (i.e. schools, health centers, DYS, DCF, DMH, state court system, detoxes, etc)

Education and Experience Required:

A Master’s degree in an applicable field required.  A minimum of two years post-graduate experience within a multidisciplinary treatment setting.  Full license/certification preferred or as required for core staffing complement.  Must have or obtain Full Certification in A-CRA/ACC Evidence Based Practice within 6 months of hire.  Obtain A-CRA/ACC supervisor certification within 8 months of hire. Two years experience working with adolescents.  Driver’s license required.

Title Senior Residential Counselor

Reports to the Program Coordinator. Responsible for the health, safety and quality of life of individuals in a residential setting through advocacy, teaching, and service planning

Essential Duties and Responsibilities:

  1. Supervise and/or participate with individuals, based on their needs in self-help skills, activities of daily living, socialization, leisure time and community integration.
  2. Assist the individual’s team to develop the PSTP/ISP.
  3. Implement, teach, and assist individuals with PSTP/ISP goals and objectives on a consistent basis.
  4. Document and maintain all individual records related to the PSTP/ISP process.
  5. Ensure proper administration and documentation of all medication.
  6. Interface and liaison with DMH Case Manager.
  7. Schedule or assist individuals in scheduling routine and preventive medical appointments, obtain and disseminate all reports to appropriate team members.
  8. Assist individuals with mail and follow-up activities related to bills, insurance SSA/SSI re-determination and any other official correspondence.
  9. Assist individuals in planning and scheduling vacations, trips, travel, and assist consumers in money management according to PSTP/ISP objectives and/or rep-payee instructions.
  10. Monitor medications, distribute medication, assist individuals in self-medication, and document medication distribution process in accordance with agency policies and procedures and licensing requirements.
  11. Inventory or assist individuals in inventorying possessions, seasonal clothing, and personal supplies on a regular basis. Assist consumer with personal needs shopping.
  12. Participate in upkeep and maintenance of program.
  13. Attend all required trainings within prescribed timelines.
  14. Assume Medication and Human Rights Officer responsibilities.
  15. Assist the Program Coordinator in the training of new staff in the area of Service Planning and routines of the program
  16. Other duties as assigned.

POSITION REQUIREMENTS:

High school degree or GED required; Bachelor’s in related field preferred.  One (1) to two (2)  year’s successful experience, as demonstrated through a quality work product, with North Suffolk’s service planning system. Must be MAP certified, CPR/first aide certified within five (5) continuous months of employment/date of hire.

The In-Home Therapy (IHT) Clinician provides medically necessary care in a structured, consistent, strength-based therapeutic relationship with a youth and his/her family. Interventions are designed to enhance and improve the family’s capacity to improve the youth’s functioning in the home and community and may prevent the need for the youth’s admission to an inpatient hospital, psychiatric residential treatment facility or other treatment setting. The In-home Therapy Clinician may provide services as part of a team, pairing with a paraprofessional.  The In-Home Therapy Clinician performs comprehensive, home-based, psychosocial behavioral health assessments, develops youth and family-centered treatment plans, reviews and develops risk management plans, and implements focused interventions and behavioral techniques in collaboration with the youth, parent, guardian, and designated caregivers.

Position requirements:

Master’s degree in a relevant field. MA License (LCSW, LICSW, LMHC) preferred. Experience in providing home and community based intervention services preferred. Certification in the Massachusetts version of the CANS.  Bilingual ability preferred, Spanish ability highly preferred.  Must have valid drivers license, reliable transportation, and be comfortable working in the community.

The ACRA/ACC Clinician and Care Coordinator works as part of the Teen TEAM program.  The Teen TEAM program provides substance abuse, community-based treatment to at-risk, underserved adolescents and young adults who have co-occurring substance abuse and mental health diagnosis. Treatment and interventions are in conjunction with the family, care providers, natural supports and case managers from child serving programs, and the ACRA/ACC Clinician is responsible for developing treatment plans and risk management/safety plans supporting the achievement of goals identified by the client and caregivers.  Interventions are designed to be intensive and immediate (after placement or incarceration) to improve the family’s ability to function as a unit both in the home and in the community. The Clinician/Care Coordinator will be trained in CANS, GAIN, and A-CRA/ACC evidence based practices and tools to be used to promote positive social activity, positive peer relationships, improved relationship with family, and improved recovery environment and abstinence from illicit drugs and alcohol.

Master’s degree required.  Experience with substance abuse, LGBT youth, and wrap-around care models preferred.  Must have valid drivers license and be comfortable working in the community. Bilingual Spanish preferred.

This position also assists the family with addressing the youth’s emotional and mental health needs. Phone contact and consultation are provided as part of the intervention.
