This member of the CBFS Residential team  is responsible for the health, safety and quality of life of persons in an intensive group living environment through the provision of person centered/recovery oriented interventions.  Supervise and/or support residents, in the areas of activities of daily living, socialization, use of leisure time and community integration.  Assist persons with psychiatric disabilities in the development of their IAPs.  Provide rehabilitation and supportive interventions that are based in the persons’ IAPs.  Provides support and interventions that are grounded in the philosophy and techniques of psychiatric rehabilitation.  Schedule or assist persons in scheduling routine and preventive medical appointments and obtain and disseminate all reports to appropriate team member.  Administer medications in accordance with MAP regulations.  Participate in upkeep and maintenance of program, including cleaning of the facility.

Medication, CPR/First-Aid certification or ability to receive certifications required within three months of hire. At least three (3) years experience in human services required and management experience in human services preferred. .. Previous experience with provision and/or management of psychiatric rehabilitation services preferred.

Participation in the ‘interdisciplinary team’ – the function of which is to collaborate around helping each person served to reach his/her goals.

Reports to the Program Coordinator. Responsible for the health, safety and quality of life of individuals in a residential setting through advocacy, teaching, and service planning

Essential Duties and Responsibilities:

  1. Supervise and/or participate with individuals, based on their needs in self-help skills, activities of daily living, socialization, leisure time and community integration.
  2. Assist the individual’s team to develop the PSTP/ISP.
  3. Implement, teach, and assist individuals with PSTP/ISP goals and objectives on a consistent basis.
  4. Document and maintain all individual records related to the PSTP/ISP process.
  5. Ensure proper administration and documentation of all medication.
  6. Interface and liaison with DMH Case Manager.
  7. Schedule or assist individuals in scheduling routine and preventive medical appointments, obtain and disseminate all reports to appropriate team members.
  8. Assist individuals with mail and follow-up activities related to bills, insurance SSA/SSI re-determination and any other official correspondence.
  9. Assist individuals in planning and scheduling vacations, trips, travel, and assist consumers in money management according to PSTP/ISP objectives and/or rep-payee instructions.
  10. Monitor medications, distribute medication, assist individuals in self-medication, and document medication distribution process in accordance with agency policies and procedures and licensing requirements.
  11. Inventory or assist individuals in inventorying possessions, seasonal clothing, and personal supplies on a regular basis. Assist consumer with personal needs shopping.
  12. Participate in upkeep and maintenance of program.
  13. Attend all required trainings within prescribed timelines.
  14. Assume Medication and Human Rights Officer responsibilities.
  15. Assist the Program Coordinator in the training of new staff in the area of Service Planning and routines of the program
  16. Other duties as assigned.

POSITION REQUIREMENTS:

High school degree or GED required; Bachelor’s in related field preferred.  One (1) to two (2)  year’s successful experience, as demonstrated through a quality work product, with North Suffolk’s service planning system. The Clinical Coordinator is responsible for ensuring the coordination of all clinical and rehabilitative services for people in the Community Based Flexible Supports (CBFS) Program. This leadership position provides clinical supervision and guidance for a team of outreach staff and specialists or residential program staff. The Clinical Coordinator also works closely with North Suffolk staff, other service providers, guardians, family members, community contacts, and Department of Mental Health staff.  The person in this position leads a team’s efforts to provide care coordination involving collaterals from area hospitals, the Department of Mental Health, court clinics, substance abuse treatment programs and housing agencies. The CBFS Program uses a person-centered, strengths-based approach employing robust, evidence based practices to uncover motivation while working with the client to cultivate positive growth.  Meeting individually with clients and employing techniques such as motivational interviewing, the Clinical Coordinator works collaboratively with the client to assess needs and hopes while designing and implementing a highly personalized treatment plan to help clients achieve Recovery. This position requires excellent verbal communication, writing, and organizational skills and the ability to work independently in the community. Extensive training is provided and ongoing training is available and encouraged.

LPHA licensure required. Master’s Degree in Social Work or Psychology or related field preferred. Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) strongly preferred. Previous experience in the provision of clinical or rehabilitative services to adults with chronic and persistent mental illness. Previous supervisory experience preferred, but not required.
