Chelsea Community Navigator

Ready for a career with purpose? NSMHA is looking for someone with experience with Chelsea residents in recovery or actively using substances.

The Community Navigators are responsible in providing a range of mobile, navigation and care coordination to assist Chelsea residents and their families in locating resources needed to live successfully in the community. Services may include advocacy, coordination of treatment, outreach, assistance with basic needs, skill building and facilitating linkages with other community based and natural support systems to support recovery.  Liaison to community leaders, health officials, criminal justice, and school department.

Essential Responsibilities:

  • Establish and maintain trusting relationships with Chelsea residents and their families in recovery from or actively using substances.
  • Provide active efforts to engage community members who have traditionally been difficult to engage.
  • Develop relationships with providers for referrals to and from the program including detox, residential and outpatient treatment facilities, hospitals and health care setting as well as first responders.
  • Conduct assessments on individual needs, readiness and service interest to establish the appropriate referrals and linkages.
  • Links participants to wellness and recovery information and resources.
  • Complete all required documentation.
  • Provide on-call coverage services as required.
  • Work with individuals to decrease barriers to services including accompanying and or arranging program access including education and assistance with section 35 process.
  • Provide participants with harm reduction and overdose prevention education and information.
  • Complete all required training within mandated timelines.
  • Transport (drive) participants throughout the community to attend to medical and dental appointments, shopping needs, court appearances, DCF visits, important errands, etc…
  • Coordinate with the Manager of Community Leadership Team (CLT) to promote public awareness and acceptance. Provide prevention education and activities in community events and school settings.
  • Participate in Chelsea Drug Court and Chelsea Night Lite with Chelsea Probation and Chelsea Police.

Requirements:

Experience: Previous experience with Chelsea residents in recovery or actively using substances preferred

Skill: A valid driver’s license and functional vehicle required.

Education: A Bachelor’s degree/Associate’s degree preferred. LADC or CADAC preferred but relevant experience will be considered
